About the Program

Many things that affect our health cannot be treated by doctors or medicine alone.

Feeling lonely or isolated, financial stress, housing concerns, caring responsibilities, or a lack of social support can all impact our health and wellbeing.

Our Wellbeing Clinician can help connect you with local services, groups and activities that support your health and wellbeing.

This may include:

Social prescribing recognises that health is influenced by more than medical care alone.

By connecting people with community resources and supports, it can help improve wellbeing, reduce isolation and support better health outcomes.

Accessing the Program

To access this program you must be a current patient at the Adelaide Road Clinic, Gawler or Willamstown Medical Clinic.

For referrals into the service please speak with your GP, nurse or the clinic’s administration and reception team when you visit the clinic.
